NAHRO Professional Development Training and Certification Invoice Request Form

Thank you for your interest in a NAHRO Professional Development Training and/or certification exam. To invoice an individual for the training and/or certification exam, please review the policies prior to completing the form below:

• Training Invoices must be paid within 30 days of receipt.
• If payment is not received for training, continuation education units (CEUs) will be revoked until payment is received. The candidate will also not be eligible for certification until payment is received, if applicable.
• Certification Exam Invoices must be paid within 30 days of receipt.
• If payment is not received for certification exam, candidate will not be eligible for certification until payment is received.
• By submitting this form, you agree to NAHRO Professional Development’s Cancellation Policy.
